Propafenone, a new antiarrhythmic drug and also the R-(+)-isomer of propafenone N-oxide, was studied for its antiarrhythmic properties in an in vitro model for ischaemia-induced arrhythmias in guinea-pig heart. A dose-dependent reduction in frequency and amplitude of the fibrillation-like ventricular ectopic impulse induced by ischaemia was shown. Propafenone reduced the ventricular ectopic impulse in a concentration range of 10(-5) M to 10(-3) M. In the ischaemic guinea-pig heart, propafenone had significant antiarrhythmic properties even at 10(-5) M concentration (Emax: 66%). The EKG pattern of the fibrillation-like ectopic impulse was also unaffected by propafenone. The antiarrhythmic activity of propafenone (10(-5) M) was antagonized by prior exposure of the heart to the concentrations of epinephrine that induced the arrhythmia. However, propafenone (10(-4) M) had no effect on adenosine-induced arrhythmias.
